You will be automatically enrolled into a free trial for ESPN+ or something like that; you need to cancel in the first month to avoid charges for that. Otherwise everything works the same.

Should be fine - But I saw the regular season on MLB.TV was $199.99 - when I re-signed up through ESPN+ it was the $139.99 price. I think I only paid $149 last year through MLB.TV

So - I just looked - they have 3 packages
Cardinals TV - $99 - no blackouts - only Cardinals games.
MLB.TV plus Cardinals tv - $199.00 - no blackouts for Cardinals games
MLB.TV - $149.00 - blackouts apply - i guess I got that one through ESPN+
